Medical Procedures at School
If your child requires special medical procedures at school, such as: care for diabetes, asthma, gastrostomy tubes, etc, you and your physician must complete the following forms prior to the procedures being performed at school and meet with your School Nurse to make sure that all of your child's needs are met and they can be cared for safely at school.

Medications at School
If a child requires medicine at school, whether prescribed or over-the-counter, it is required by California Education Code that the following form be completed by both the parent and the physician. If you have any questions, please contact your School Nurse, or Health Services at 631-5895.
