If you need to add a dependent who is not currently enrolled on your plan, please complete the SISC membership change form. This will automatically enroll your dependent to your Medical, Prescription, Dental, Cancer, Vision and Life Insurance plans. You must include a copy of your spouse’s marriage certificate and/or child’s birth certificate along with their social security number. NEW REQUIREMENT: The first page of the most current Federal Tax Return and a copy of the marriage certificate are required for proof of spouse eligibility.
If you would like to make any changes to your current dental plan, please click on the appropriate link below, print and complete the appropriate form. To review dental insurance plan options, click on the Dental Insurance link on the right side of this page.
Don’t forget to return all completed enrollment forms into Health Benefits by August 23, 2024.
If you are an employee working less than 4 hours per day or a substitute employee, a basic Health Plan option will be offered during Open Enrollment. Click on the “Affordable Act- Bronze Plan” icon located on the home page.

New employees have 30 days from date of hire to sign up.
When does coverage begin?
The 1st day of the next month after hire date.
