Since 2014 The Guardian Life Insurance Company of America has been providing you and your family with benefits for your Basic and Voluntary Supplemental Life Insurance. Life Insurance is included in the employee health benefit package. Every insured employee under the age of 65, is provided with a basic $50,000 policy. Every insured dependent is provided with a basic $5,500 policy. Please refer to the plan summary regarding exclusions and limitations.
If you would like to add Voluntary Supplemental coverage to your policy for your dependents, you will need a complete the attached Evidence of Insurability document and return it directly to the Guardian.
