Extended Learning-Parent Information:

OPTION 1

  1. Pick up a registration form at your student’s school.
  2. Complete one form per student that you want to enroll.
  3. Return the completed form to your school site.
  4. Wait for notification of your student’s enrollment*

OPTION 2

  1. Open link below, fill out the Registration form and Photo Release form.
  2. Complete one form per student that you want to enroll. Print form.
  3. Bring the completed for to you school site.
  4. Wait for notification of your student’s enrollment.*

*Once the program is filled to capacity, students will be placed on a waiting list and admitted as openings become available. Priority for enrollment from the waiting list is made for students that need extra help in school and for children whose siblings are already enrolled in the program.
